New enrollees need to fill out Annex 2: Individual Participation in Urban Residents Serious Illness Medical Insurance Information Registration Form, whichThe insured manual number must be filled in carefully for parents to log in to the Beijing Social Insurance online service platform to obtain the verification code and input the card and discount information for payment, and query the insurance information。(Please log in to the social insurance online service platform before October 24 to input bank card and discount information, so as to facilitate bank deduction) For specific procedures, please see Annex 3: Parents Online Modification of debit information Operation Guide.

At the same time, the following materials should be provided according to the situation:

A Students registered in the city

Students in this city who participate in the serious illness medical insurance for students for the first time should provide a copy of the home page and the personal page of the household register, and a copy of the change page should be provided if there is a change page。In line with the standard of the second generation ID card, a one-inch white background free color photo and other related materials。

B Students who are not registered in the city

Those who meet the six categories of students who are exempt from loan fees are also required to provide relevant supporting materials (see Annex 4).

C Students who need to send Social Security identification numbers

For the first time, the overseas Chinese children of the six categories of people who are exempt from borrowing fees, the children of Taiwan businessmen who invest in Beijing, and the Hong Kong, Macao and Taiwan students whose parents have a Beijing account, because there is no 18-digit ID number, they need to apply for the first layer of the Social Security Center in the district。Materials required for dispatch numbers (see Annex 5)。Deadline for submission of all materials: November 15
